Device and activation fees: Upon your completed order form being received, you will be charged a one-time fee of $99.00
plus applicable taxes for the SIMpill® pill bottle device and its set-up and activation by Clinical Technology Advisors, Inc.
(CTA).
Monthly service fees: Upon set-up and activation, your credit card will be charged a payment of $174.00 plus applicable
taxes for the first six months of service. Your credit card will be charged $29.00 per month plus applicable taxes thereafter
for continued compliance notification service.
Cancellation: A $250.00 cancellation fee will be charged to your credit card if you cancel your SIMpill compliance
notification service before the twelve month service period expires. You may cancel your SIMpill compliance notification
service anytime after the initial twelve month service period ends without penalty.
Shipping and handling: Free USPS Priority shipping and handling is included with your order.
Return, Refunds, Exchanges: An item must be eligible for returns. Returns are accepted within 30 days of receipt by the
customer of the item - sorry, no exceptions.
Before returning an item customers must request an RMA # by calling 1-888-4-SIMPILL. Items will only be accepted if
returned in the condition that they were received by you, with all parts and original packaging included. The customer is
responsible to insure that we receive the return. Please keep all tracking and documentation once you ship your item back
so it can be referenced if necessary.
Returns can take 7-10 days to process once it has been received by CTA. Shipping costs back to CTA is the
responsibility of the customer. In the event of a replacement for a DOA item, shipping the replacement back to the
customer will be free of charge.
If the above conditions are not met, CTA reserves the right to charge up to a 25% percent restocking fee.
Remember, as with any medication compliance program, always work with your doctor, nurse, or pharmacist to develop
a long-term compliance management solution that works for you.
